Need to postpone, cancel your membership, or change an item such as your billing information, address, or phone number?  Find the form (s) you need and send an email request for it. For any additional questions, please check out our faq's.

The cancellation form is required if you wish to terminate your membership.  To be eligible, you must have a current account (accounts that are past due/in collections must be paid before cancellation).
 
You must give a 30 day notice before the next billing date and specify the reason you are canceling on the form.

You may either bring your form in person to the club or send via US mail.  If sending us a cancellation form, you must do so first class mail with return receipt.  Faxes are not accepted. We strongly recommend that you follow up with us to make sure your request was processed.

The freeze form is for members who wish to freeze their memberships from 1 up to 3 months (6 months in some instances). Freeze forms may be faxed. We strongly recommend that you follow up with us to make sure your request was processed.

Memberships can be frozen for a period of 3 months for personal reasons at a cost of $10 per month. For a medical reason or military deployment, memberships can be frozen up to 6 months at no cost with either a doctor's note or proof of deployment.

The change form is for members who need to update their current billing method, address, phone, or other important information related to their membership. (If your form contains credit card or bank account info, you must complete this form in person) .Faxes are not accepted.  We strongly recommend that you follow up with us to make sure your request was processed.
