If sending us a cancellation form, you
must do so first class mail with return
receipt. Faxes are not accepted.

The freeze form is for members who wish to
freeze their memberships from 1 up to 3 months
(6 months in some instances).
Freeze forms may be faxed.
We strongly

Memberships can be frozen for a period of 3 months for personal reasons at a cost of $10 per month. For a medical reason or military deployment, memberships can be frozen up to 6 months at no cost with either a doctor's note or proof of deployment.

The change form is for members who need to update their current billing method, address, phone, or other important information related to their membership. (If your form contains credit card or bank account info, you must complete this form in person) .Faxes are not accepted.
